SECTION 00050 AGREEMENT

NOTIFICATION OF ACCEPTANCE OF USE OF GENERAL PERMIT

Attached is Addendum No. 1 and supporting documents to the Thomas Drive & Beach Drive Utilities Improvements - Phase I Project. These forms MUST be signed and submitted with the Bid Form. 

Bids will be received until 2:00 PM Central Time, Tuesday June 18, 2019 at City of Panama City Beach City Hall, 110 South Arnold Rd, Panama City Beach, Florida and will be opened and read publicly immediately thereafter. All Bids shall be submitted in an envelope clearly marked “Sealed Bid – Thomas Dr and Beach Dr Utilities Improvements – Phase 1”.   A Bid Bond in the amount of 5% of the total amount of the Bid shall accompany the Bid.  The City of Panama City Beach (“City”) reserves the right to reject any and all Bids.  All Bids shall be firm (including all labor and material prices) for a period of 30 days after opening. 

This Addendum forms a part of the Contract Documents and modifies the original Bidding Documents, dated May 13, 2019 as noted below. Acknowledge receipt of this Addendum in the space provided on the Bid Form.  Failure to do so may subject Bidder to disqualification.

1.1  CHANGES TO PROJECT MANUAL AND SPECIFICATIONS

A.      Specification Section 00010 -Advertisement for Bids. REVISE bid opening date to REFLECT

"Bids will be received until 2:00 PM Central Time, Tuesday June 18,  2019"

B.      Specification Section 00020 -Information for Bidders. REVISE bid opening date to REFLECT 

"Bids will be received until 2:00 PM Central Time, Tuesday June 18,  2019"

C. Specification Section 00050 - Agreement was inadvertently omitted from the Bid Documents and had been included with this Addendum (see attached).

D. Specification Section 00090 - Notice to Proceed. REVISE Substantial Completion of Work To REFLECT:

1. Milestone No. 1 (Phase 1-A): 60 Consecutive Calendar Days from Notice-to -Proceed Date.

2. Milestone No. 2 (Phase 1-B): 135 Consecutive Calendar Days from Notice-to-Proceed Date.

E.     Specification 00099 - Certificate of Insurance form was inadvertently omitted from the Bid Documents. A copy of the certificate has been attached to this Addendum.

F.      Appendices "D" (Release and Affidavit from Contractor) and "F" (Release and Affidavit from Subcontractor) were inadvertently omitted from the Bid Documents and had been included with this Addendum (see attached).

G.      FDEP Notification of Acceptance to Use General Permit. A copy is attached to this Addendum.

H.      FDEP Notification of Acceptance to Use General Permit. A copy is attached to this Addendum.

I.      Bay County Use of R-0-W Permit

J.      Attached to this Addendum are AT&T markups of their utilities along Thomas Drive provided by Bay County.
